New Patient visits are scheduled for two hours and will include a comprehensive review of:

Insurance verification is required before scheduling the fist patient visit.  The following information is needed in order to verify insurance: 

Picture or Driver’s License or valid ID

Picture of All Insurance Cards including numbers

SS#

Current Address or Address of where they are moving to- (Example: Assisted Living Facility)

Point of Contact AND Emergency Contact Person- Including phone number and address

Please address if the patient can speak for themselves

Current notes or H&P if applicable

**Only under very urgent circumstances can we try to verify and see patients’ same day**

We do offer Self-pay visits at $150 each visit 

Medical history, current conditions, 3 top concerns, current medications and supplements, social history, current living situation, extended social support, work history, family history, (these could be listed in bullet points, and perhaps in alphabetical order)Please print out the two attachments and fill out prior to your first at home visit.
